Grappling in Paradise LLC – Waiver of Liability

Before you make your purchase to attend our camp, please review and sign. (Event: Grappling in Paradise, Oregon June 5th – 9th 2024)

Acknowledgement of Risks, Injury & Obligations:

I acknowledge that the activity I will be engaging in is a dangerous activity and that by participating; I am exposed to certain risks. I assume the risk and responsibility for any injury, death, or property damage resulting from my participation, and assume all risk and liability for myself or my minor child during the stay of this event on and off premises.

I acknowledge and understand that while participating in this activity:

• I may be injured physically.
• I may cause injury to other people.
• I am responsible for my personal property.
• Other people participating in the activity may cause injury to me or damage to my property.
• The conditions in which the activity is conducted may vary without warning.
• “Grappling in Paradise LLC” and all of its Instructors, owners, and employees are not responsible for damage or injury to your personal property at any location at any time during this event.
• Instructors, Owners, Employees, Volunteers, and Property Management are not responsible for injury, or death caused on-site or off-site at any time during the entirety of the event.
• Any activity I engage in off-premises will be at my personal choice and discretion, I will hold Grappling in Paradise LLC, its Owners, Employees, and Volunteers liable for any accident, injury, or loss during such activities. (For example Excursions).
• Ages 13 – 17 years old ONLY with parent/guardian present.

 

Release and Indemnity:

I participate in the above activity at my sole risk and responsibility. I indemnify and hold harmless Grappling in Paradise LLC, and any Owners, Employees, Volunteers, or Instructors teaching on behalf of Grappling in Paradise LLC, against any and all claims, damage, demands, rights of action, or causes of action, of any person or entity, that may arise from injuries or damages sustained by me or my guests.

 

Guidelines to Follow:

• Appropriate clothing, footwear, protection, and gear must be worn at all times.
• Each member and their guests must respect others and behave appropriately at all times.
• Grappling in Paradise LLC, its Owners, Employees, and Volunteers will not at any time provide alcohol to any participant and will not be responsible for consumption from any participant whether a minor or adult during this trip.
• Hygiene is important. Communicable diseases spread easily in a training environment. Do yourself and others a service by not bringing unwanted germs into the training camp.
• No shoes on the mat – no exceptions, all class rules/etiquette must be followed
• Keep fingernails, toenails short, no makeup, glitter, or strong perfume that can rub off on other people or the mats.
• Come to class clean, shower, or clean off after work before stepping on the mats.

By signing this form, you acknowledge that you have read, understand, and agree to the above statements.

• Cancellations Within 30 Days of Booking and Prior to January 28th, 2025 are eligible for a full refund minus card processing fees.
• Cancellations Within 30 Days of Booking and between January 29th, 2025 and March 1, 2025 are eligible for a 50% refund. 
• Cancellations after March 1, 2025 regardless of when they were Booked are non-refundable. Credits may be applied to a future camp or transferred to another camp attendee.
